Registration

We now offer multiple ways to register your groups. Below are links for the downloadable PDF and the digital Google Form.

PDF forms can be emailed to educator@midwayvillage.com or mailed to 6799 Guilford Road Rockford, IL 61107 ATTN: Education.

Deadline to register is Wednesday, September 18th.

Deposit/Balance

A non-refundable 50% deposit is due by Wednesday, September 17, 2025. Payment is due in full by September 24, 2025. Payment options are:

1. Credit Card can be processed over the phone

2. Mail check to 6799 Guilford Rd Rockford, IL 61107 ATTN Education

3. Pay at the Museum Center in advance
